You want to receive requests from your website visitors with files attached? Then you are at the right place. Let your customers upload any files to their request. You can use the drag and drop builder to determine what information you get from your requesters. Name, email, comments, dropdown, checkboxes and of course the upload field. There are many use cases where a file .png, .jpeg, .pdf and many more of the requestor is helpful. Strange feature
Strange! I don't know if it's WIX (which by the way needs updating a lot) or if it's the app. But when we add it to the page, it ends up in the middle of the first page. It's VERY strange, of course it should be added in the back office, then I should be able to choose in the catalog which products I want it to be used for. Change it and do it right!

Hey, this is a form builder to receive files, if you need an upload button showing on specified products and sending the files along with the order, you should consider using Uploadly, instead of Formly.

Maybe it works now?
This app initially would not download so the technical team helped and got it downloaded. Then it would not load the form to the widget and the technical team had no solution to the problem. Therefore, it was deleted and removed and I was refunded. A week later I tried it again and it seems to be working fine with no issues.

Responses are nowhere to be found!
I installed this app to create a file upload form. However, I couldn't find a single element that enables me to view the form submissions.

Very good technical service
I had an issue with the automatic translated French version of the app and the technical service fixed it very quickly. They answered my email request within the hour!

Problem with multilingual and no customer support
I have installed this App on my website. It is easy to set up the form in French or English but it does not seem possible to set up the form in both language on my multilingual website, using WIX Multilingual. I tried many times to contact the customer support for the App but never got a reply.
